How I Use Berberine in my practice?

As a board-certified endocrinologist, I've integrated the powerful plant extract, berberine, into my practice to enhance diabetes care and metabolic health.

What is Berberine?

Berberine is derived from plants such as goldenseal and barberry, and it's celebrated for its ability to improve insulin sensitivity and mimic the effects of metformin, a common diabetes mellitus medication.

How does Berberine work?

Berberine activates the AMP-activated protein kinase (AMPK) pathway, central to regulating metabolism and reducing glucose production by the liver—a process known as gluconeogenesis. By enhancing the insulin uptake by cells, berberine helps stabilize blood sugar levels, making it a valuable supplement for individuals with insulin resistance, prediabetes, and type 2 diabetes, as well as conditions like polycystic ovarian syndrome (PCOS).

Clinical application and dosing
In my practice, I recommend starting with 500 mg of berberine, potentially increasing to two or three times a day, depending on the patient’s response, absence of any intolerances, and specific health needs. In patients with diabetes mellitus type 2, monitoring blood glucose levels is an essential part of my care. While berberine is not a replacement for prescribed medications, it serves as a complementary approach for patients looking to manage their diabetes -2 with non-pharmacological options or evidence-based supplements and plant extracts.

Long-term use and patient guidance

There's no definitive study on the long-term use limits of berberine, but consistent positive outcomes are often observed in studies. Patients should not self-prescribe but rather discuss berberine with their healthcare provider to determine if it's right for their health strategy. I also recommend avoiding use of both metformin and berberine at the same time given their similar mechanism of action but very different potencies. There has not been a randomized control trial using both these for comparison.
