1. Please call/email us for INITIAL NEW PATIENT CONSULTATION appointments. We do not need any referrals. We do our best to accommodate for the same or the following week's appointment based on urgency. A non-refundable $50.00 deposit is required to schedule a consultation which will be counted towards the consultation fee.
2. We are a Direct Specialty Care Office, Dr. Makhija does not bill your health insurance. Please read FAQ for details.
3. SELF-SCHEDULE FOLLOW-UP APPOINTMENT is for established Patients/Members.
4. SELF-SCHEDULE NEW PATIENT CONSULTS are for Referrals from Dr. Lifestyle Clinic patients ONLY

PLEASE NOTE: Our office does not file insurance claims of any type. Our fees are affordable without using insurance, and you never have to worry about our services if your insurance plan changes at the beginning of each year. We offer these reduced fees while providing excellent service and top-quality medical care. We do not sign contracts with insurance companies or managed care plans that force us to have their interests ahead of our patients. You are our highest priority, not an insurance company or a governmental agency. Your health information never leaves our office without your expressed request to do so (unless requested by legal authorities)
